Program Details
One free badge per household. Offers may not be combined. Offer does not include installation labor costs. A completed claim form must be mailed in or filed online. No facsimiles or photocopies of the claim form will be accepted. Claims that are lost, late damaged, misdirected, incorrect, illegible, incomplete, mutilated, postage due, or that have a P.O. return address will not be honored, acknowledged, or returned. This offer is only valid in the USA and Canada. Please allow 8-10 weeks for delivery of your replacement badge. Void where prohibited, licensed, restricted, or taxed by law.
